To apply for a duplicate Oklahoma title, you must fill out an Application for Duplicate Certificate of Title for Vehicle/Boat/Motor (Form 701-7).

Once you complete this form, you must submit it to the Motor Vehicle Division or any Oklahoma motor license agency.

To obtain a duplicate title in Oklahoma, you must also submit an Oklahoma registration, or evidence of out of state residency.
